To apply for a Providence Individual & Family plan in Oregon or Washington, you must reside in the selling area for the selected plan type. See the service area lists below to find out which Providence Individual & Family plans are available in your area.

Please note that the selling area for each plan may be different from the provider network for Connect, Standard, HSA Qualified plans, and Providence Oregon Direct plans. See our health plan basics or click on the provider network links below to view the provider network for that plan.

Please note: the Individual & Family Dental plan is available in all Oregon counties and is only available to purchase directly through Providence Health Plan.

Connect plans

Connect plans are available in the counties listed below. They combine a medical home model of care with a tailored provider network.

The Connect Direct plan is only available to purchase directly through Providence Health Plan or through a producer.

Members select a medical home from our Providence Connect Network with this plan. The medical home model provides a team of health professionals dedicated to your overall well-being.

Standard, HSA Qualified & Providence Oregon Direct plans

Standard plans

Available state-wide, our Standard plan benefits are determined by the state of Oregon.

HSA Qualified plans

Available state-wide, high-deductible plans provide affordable coverage with a lower premium.

Providence Oregon Direct plans

Our Providence Oregon Direct plans are available state-wide.

The Providence Oregon Direct plan is only available to purchase directly through Providence Health Plan or through a producer.

Depending on where you live, your provider network will be either the Providence Choice Network or the Providence Signature Network. You will need to choose a medical home if your plan is on the Providence Choice Network.

Columbia plans

Providence Columbia plans combine a medical home model of care with a tailored provider network. Members select a medical home from our Providence Choice Network with this plan.
